This initial consult will take approximately 2 ½ to 3 hours as both eyes will be dilated, and our team will perform several diagnostic tests.
If you do not feel comfortable driving with your eyes dilated, please bring a driver with you to your appointment.
When your turn comes, be assured that you will be given a thorough examination and time for explanation and questions.
Your referring provider will need to submit a referral to us. Referrals can be sent to Verdier Eye Center using on one of the following ways:
After we receive your referral, we will contact you directly by phone within 3-5 days to schedule your appointment
After your appointment is scheduled, please complete the required registration forms and bring them with you to your appointment
Please note: if you are unable to fill these forms out prior to your appointment, you may fill out paper copies in our office. Please plan to arrive 5-10 minutes before your appointment.

Please understand that your initial appointment is an evaluation and consultation. You will NOT have surgery during this appointment. If a procedure is necessary, one will be scheduled during this consultative appointment.
If you plan to discuss your treatment with a relative or friend, we encourage you to have him or her with you during your exam and to be placed on your HIPAA form when you check in for your appointment.
