New Patients: What to Expect

We want to take this opportunity to welcome you to our practice and help make your visit comfortable

This initial consult will take approximately 2 ½ to 3 hours as both eyes will be dilated, and our team will perform several diagnostic tests.

Drive

Dilation causes sensitivity to light and an inability to see things well at close range

If you do not feel comfortable driving with your eyes dilated, please bring a driver with you to your appointment.

Delay

We may run late because of emergencies, delays in surgery, or the discovery of a more severe problem that can take additional time

When your turn comes, be assured that you will be given a thorough examination and time for explanation and questions.

How to schedule an appointment

Your referring provider will need to submit a referral to us. Referrals can be sent to Verdier Eye Center using on one of the following ways:

  • Online Referral: Your referring provider may submit a referral request through our website by clicking here
  • Fax: Your referring provider may submit a referral request by fax to us at: 616-949-8620
  • Michigan Health Information Shared Services (MiHIN): Your referring provide may submit a referral request through the Michigan Health Information Shared Services portal if they have this referral system at their practice

After we receive your referral, we will contact you directly by phone within 3-5 days to schedule your appointment

Step 3

After your appointment is scheduled, please complete the required registration forms and bring them with you to your appointment

Please note: if you are unable to fill these forms out prior to your appointment, you may fill out paper copies in our office. Please plan to arrive 5-10 minutes before your appointment.

What should I bring to my appointment?

You need to bring the following items to your appointment:

  • Your current insurance card and driver’s license (for billing and identification purposes)
  • Your co-payment, if applicable
  • A list of all medications you are taking
  • A list of all eye drops you are taking
  • Your current glasses or contact lenses
  • Sunglasses
  • The name of your referring doctor
Will I have surgery?

Please understand that your initial appointment is an evaluation and consultation. You will NOT have surgery during this appointment. If a procedure is necessary, one will be scheduled during this consultative appointment.

Can I bring a family member or friend?

If you plan to discuss your treatment with a relative or friend, we encourage you to have him or her with you during your exam and to be placed on your HIPAA form when you check in for your appointment.

What insurance do you take?

Our practice accepts most major insurance carriers. Please visit our insurance and billing page for a complete list of carriers

Do you offer online Bill Pay?

Yes. Through our patient portal you can pay your bill online.

What is Patient Portal?

It provides you access to your account/chart. You’ll be able to do the following online:

Refill medications
See your future appointments Please double check with our office if your appointment was canceled and/or rescheduled
Account statements

When we schedule your appointment, we will send an enrollment link from NextMD to the email you have provided. If you have not received it, check your spam or junk folder.

Visit Patient Portal

For entirely new patients to the patient portal:

  1. Click the enrollment link provided from the enrollment email You need to call the office to generate the link
  2. Give your phone number and your date of birth
  3. Create a username and password Make sure you write down your username and password somewhere safe
  4. Create your security questions
  5. You are finished!

Had a patient portal before, but can’t access it:

  1. Call our office to let us know that you need a password reset
  2. A password reset email will be sent to the email provided
  3. Our office will provide you the necessary token to reset your password

Forgot your username:

  1. Click the link that says “Forgot username” and it will ask for the email address connected to the account
